Privacy of personal information is an important principle in my work. I am committed to collecting, using, and disclosing personal information responsibly and only to the extent necessary for the services I provide. My goal is to be open and transparent regarding how I handle personal information.
This policy applies to Fully Human Psychology Services, operated by Dr. Keegan Barker, Psychologist. It covers both the personal information collected through this website and through the delivery of psychological services.

Personal information is information about an identifiable individual. It includes information that relates to an individual's personal characteristics (e.g., gender and gender identity, age, income, home address, phone number, ethnic background, family status); health (e.g., health history, health conditions, health services received); or activities and views (e.g., religion, politics, opinions expressed by an individual, or an opinion or evaluation of an individual).
Personal information is different from business information (e.g., an individual's business address and telephone number), which is not protected by privacy legislation.
This website does not use tracking cookies or analytics software. No personal information is collected simply by visiting this site.
The contact form on this website is provided through Microsoft Forms, a secure cloud-based service. When you submit the contact form, the following information may be collected: your name, email address, and phone number. This information is used solely to arrange an initial consultation and is not shared with any third parties.

I am the only staff member at Fully Human Psychology Services. I may use several consultants and agencies that may, in the course of their duties, have limited access to personal information I hold. These could include computer consultants, bookkeepers and accountants, banking professionals, and lawyers. I restrict their access to personal information as much as is reasonably possible, and I have their assurance that they follow appropriate privacy principles.
Like most psychologists, I collect, use, and disclose personal information to provide psychological services. This could include information about a client's health and mental health history, including family history, physical conditions and functioning, and social situation.
I also collect, use, and disclose information for purposes related to or secondary to these primary purposes, including:
— To invoice clients for goods or services that were not paid for at the time, or to collect unpaid accounts.
— The College of Psychologists and Behaviour Analysts of Ontario may inspect my records as part of their regulatory activities in the public interest.
— The cost of some services may be paid for by third parties (e.g., private insurance). These third-party payers often have your consent or legislative authority to direct me to collect and disclose certain information in order to demonstrate client entitlement to this funding.
I take the protection of personal information seriously. Steps I have taken include:
— Paper information is either under supervision or secured in a locked or restricted area, and transmitted through sealed, addressed envelopes or boxes by Canada Post.
— Electronic hardware is either under supervision or secured in a locked or restricted area. Strong passwords and encryption are used on all devices, and two-factor authentication is used wherever possible.
I use Owl Practice to manage personal information (e.g., name, date of birth, address, email), billing, and health information (e.g., session notes, reports, and documents related to your care). Owl Practice hosts data in Canadian-owned data centres, with encryption using SSL. For more information, visit owlpractice.ca/feature-security.
I use Microsoft for email, file sharing (SharePoint), and video sessions (Teams). Microsoft hosts data in Canadian data centres with continuous backup, end-to-end encryption, and two-factor authentication. For more information, visit microsoft.com/trust-center/privacy.
You have the right to be notified if your personal health information is lost, stolen, or improperly accessed. Should a breach occur, I will implement a breach protocol that includes steps to contain the breach, contacting the Information and Privacy Commissioner of Ontario (IPC) where applicable, and notifying you.
The College of Psychologists and Behaviour Analysts of Ontario requires that I keep clinical records for at least 10 years after the last contact date.
I destroy paper files containing personal information by cross-cut shredding. Electronic information is destroyed by secure deletion and, when hardware is discarded, the hard drive is physically destroyed.
With very few exceptions, you have the right to see the information in your file. To request access, please submit your request to me in writing. I will respond within 30 days.
If you believe there is a mistake in the information in your file, you have the right to ask for it to be corrected. This applies to factual information but not to professional opinions.
